Request For System Modification

College Information Technology policy is to implement and maintain Banner and related administrative computer systems as closely as possible to the baseline product(s) delivered by the vendor. This policy applies to any software system maintained by the Information Technology Department. Any modifications must be requested for evaluation by the Information Technology Department.

The process must be initiated by one of the data custodians after consultation with the appropriate AVP or VP. The data custodian then completes the form below.

Include a concise statement of the critical business need for the modification, the work that cannot be done without a modification and other options considered to address the issue. Also include analysis that includes difficulty of making and maintaining the modification, possible impact on other systems, other options for addressing the need both within the system and outside it.
