Parent Referral Service and Billing Form

Thank you for giving us the opportunity to assist in your child care search in Lane County, Oregon. We understand your need to find quality, affordable child care for your child or children.

The subscription fee entitles you to customized referrals, consultation and resource materials for one full year. The responsibility to screen and select a child care provider remains with you, the parent or guardian.

Lane Family Connections attempts to offer you five to seven referrals per call. If three or less referrals are given your fee will be reduced; no referrals will eliminate fee.

How it works

1) Determine your fee based on household size and annual household income.

2) Payments may be made by Visa, Mastercard, Money Order, personal or Cashier's checks. Make checks payable to: Lane Family Connections.

3) Complete the Parent Referral form and send it either by fax or regular mail.

4) Be prepared to let Parent Consultant know your payment method. If choosing a credit card, have the number and expiration date ready.

I. Basic Service – Sliding Fee Scale (See below)

• Referrals to providers who have been updated within last 3 months
• Summary printout includes:
• First and Last Name (some counties would only list first name)
• Phone number
• Street Name, City, Zip code
• Type of care, Regulation status
• Days and Hours
• Environmental Factors
• One reference publication
• Valid for one year
