LCC Nursing Integrative Practicum Program (formerly Preceptor Program)
Evaluation Process
- Review the student evaluation tool and questioning strategies prior to the start of the preceptor experience.
- The preceptor student evaluation is divided into 6 major objectives (designated as A-F). These objectives relate to the 3 major RN roles identified by LCC faculty. The three major RN roles are:
Care Provider - this role is divided into 4 objectives which are all evaluated separately
Manager Role
Member of Discipline
- The preceptee student should be evaluated in their attainment of each of the major objectives throughout the term.
- Record examples of the student’s behavior that demonstrate how he/she met or did not meet each of the 6 objectives on the 5x7 evaluation cards provided in your preceptor notebook.

- Complete an evaluation card for each group of 3-4 days that the student is with you. Use the following guidelines for assigning a grade:
PASS:
The student initiates and performs the nursing activities described in the objectives or the course within a reasonable time period and with occasional supportive or directive cues.
For the purposes of formative (ongoing) evaluation throughout the quarter, the following scale is added. This scale will not be used in summary (final grade) evaluation.
P-1 Pass: independent, correct, without supportive cues
P-2 Pass: correct, with supportive cues
NEEDS IMPROVEMENT:
The student needs direct guidance and instructions in to complete care within a reasonable time frame and to prevent error or injury to patients when performing the nursing activities described in the objectives for the course.
NO PASS:
The student fails to demonstrate course objectives safely. Is unable to demonstrate, understand, and apply the nursing process and scientific rationale to nursing interventions.
MIDTERM
Complete a mid-term evaluation conference with the student after approximately 8-9 days. Summarize clinical behavior in writing on the Mid Term and Final Preceptor Evaluation.

The student will complete a self-evaluation and bring it to the mid-term and final conferences. Notify faculty liaison for any NI or NP areas. Faculty can be present for mid-term evaluation if you desire. Faculty will assist you to develop a plan for behavior change for the student if needed.
FINAL
Complete a final evaluation during the last 3 days of the preceptor experience. Schedule the meeting time so faculty liaison can attend final evaluation. Discuss with instructor liaison any areas of concern or any NI or NP grades prior to the evaluation with the student.
