Point Based Application. Course or degree completion is not a requirement to apply, but will be used to prioritize applicants. 3 extra pts given for prior degree. Additional points will be given for Paid Work Experience in a dental office with a Minimum of 415 hrs. (Must be verified by pay stubs or W-2 forms.) |

Enrollment in this program is limited to 30 students per year.
- Applicants must be Oregon residents.
- Applicants must have a high school diploma or a GED by the start of Fall Term 2008.
- Evidence of a physical examination (within the previous nine months) must be provided prior to admission to the program.

Program expenses including tuition plus differential pricing are approximately $7451. Required student uniform and student issue, lab fees, and National Exam fees can range an additional $2500, for this nine month program.
