Family Planning Expansion Project (FPEP)** The Lane Community College Health Clinic participates in FPEP, the Family Planning Expansion Project. This federal program provides FREE family planning services, birth control and reproductive health care to men and women. Our on-line FPEP Guide, located below, should answer most of your general questions regarding the program. The Enrollment Form is required. Forms are available in English or in Spanish:
Note: If you were born in Oregon and would like the Health Clinic to try to verify your citizenship electronically, please print and fill out the Oregon Birth Information Form (.pdf)*. This form will be used to search for your birth records through Oregon Vital Records and can take several months. If an electronic match is made, you will not need to provide your original birth certificate. To make an FPEP appointment, please call the LCC Health Clinic at (541) 463-5665 or visit the Health Center in person.
