Ability to Benefit – Complete this form to verify you have a high school diploma or GED certificate. If you do not, and you are at least 18 years old, you may take the Ability to Benefit test in the Testing office. Testing staff will complete the form for you. |

Appeal To Reinstate - If your financial aid status is "suspended", use this form to explain circumstances that prevented you from meeting Satisfactory Academic Progress Standards. |
Credit Limit Request For Review – Complete this form if you have been notified you may exceed the maximum credit limit. |
Request For Change – Use this form for miscellaneous requests to Financial Aid, such as enrollment changes, cost of attendance changes, accepting previously declined loans, or to request copies of letters previously sent. |
Request To Apply For Financial Aid Without Parent Information – Dependent students may use this form if requesting to be considered an Independent student. See Dependent vs. Independent to determine if you may be eligible. |
Title IV Release - This link will return you to your ExpressLane account, where you can complete the on-line release by choosing "My Financial Aid Eligibility Requirements" on the main Financial Aid menu. |
