Getting Started with Disability Services
THE FOLLOWING IS A CHECKLIST OF ACTIVITIES FOR YOU TO COMPLETE
IN ORDER TO RECEIVE SERVICES THROUGH OUR OFFICE:
Complete the college admission process and get your L number though the
Enrollment Services
office.
Take the
College Placement Tests
(3 - reading, writing & math) at the Testing Office, and bring the results to Disability Services.
Make an appointment to attend a Disability Services Orientation session.
Collect and send for documentation of your disability.
Make copies of DS Application and all documentation.
Submit DS Application, supporting documentation, and documentation releases to Disability Services.
Follow-up with documentation providers as needed.
Attend your DS Intake Interview. Be prepared to discuss your accommodation needs.
REPEAT THE FOLLOWING ACTIVITIES IN ORDER TO
RECEIVE DS ASSISTED ACCOMMODATIONS EACH TERM:
See your
Academic Advisor
to participate in
Advance Registration
and firm-up your class schedule as early as possible.
Register for classes and print out your "Student Detail Schedule".
Bring a copy of your "Student Detail Schedule" from ExpressLane to DS as soon as you are registered.
Request your accommodations at the Disability Services Front Desk.
Follow the
Step by Step... Term by Term
handout to request your DS assisted accommodations.
Make an appointment with each instructor to share your Letter of Accommodation and discuss which accommodations you will require in that class.
