Disorder Documentation Requirements
Students requesting services and academic accommodations
from Disability Services must provide current documentation of their disabling
conditions. In order to establish eligibility and ensure the provision of
reasonable and appropriate accommodations, we ask for your doctor (M.D., D.O.,
or other diagnosing professional) to provide information listed below:
- DSM-IV diagnosis of ADHD, and the specific type
(including DSM IV code)
- Date of onset
- Date last seen about this diagnosis
- Diagnostic instruments used, including rating scales,
and attention tools that measure components of attention (e.g. CPT, TOVA,
other)
- Brief description of the history of this condition
(evidence of ADD/ADHD must be established in childhood with some obvious
impairment at least by high school)
- Co-morbid conditions or other explanations have been
ruled out (e.g. depression, bipolar, LD, PTSD)
- Does this condition have a substantial affect on one or
more of the student's major life activities?
- Other symptoms or effects related to the diagnosis
- Medications prescribed, and their potential side effects
that may affect student's ability to function in college
- Recommended accommodations to provide student with equal
access to the learning environment
- Additional diagnostic information recommended: cognitive
assessment (Woodcock-Johnson Battery-Revised, WAIS-R, or WAIS-III,
Stanford-Binet Intelligence Scale - 4th Edition, WISC-R or WISC-III if given
prior to age 16 and within the last 3 years)
