This is an example of the Budget Change Request form. It indicates at the top that a new number is required (1); the budget account numbers (BAN) involved in the transfer (2); the current budget (3); the expenditure amount being added--in whole numbers (4); the revenue amount being added (5); the amount being reduced (6); the revised budget (7); the totals at the bottom to ensure the numbers above balance (8); the purpose of the request (9); and signed off by the budget administrator (10). If the transfer is to be recurring, mark an R in the designated column (11).
